    You can telephone and request a Registration Pack.

    Telephone 0118 9874551 between 10am and 6pm and ask for a

    Registration Pack to be posted to you, which you can complete and send

    back to us by FREEPOST to the address above.

    Come in to our reception and ask to register. 

 

    Reception is open Monday to Friday 8am – 6.30pm.

 

   

 

    Please bring with you a copy of your I.D and proof of address

    e.g passport, photo driving licence and a utility bill or tenancy agreement

 

 

 

 

 

 

 

What happens next?

  • We will enter your details onto our computer system, and for patients who have              previously registered with a doctor in the UK arrange for transfer of your medical records to us.
  • If you need to see a doctor or nurse you can book an appointment after we have received your completed forms
  • If you take regular medication for contraception, asthma, hay fever, acne or eczema you will need to book an appointment with one of our specialist nurses at least a week before your medication runs out. (Please tell reception which condition your medication is for so they can direct you to the correct nurse)
  • All other medications please book an appointment with a doctor at least one week before your medication runs out.
